Restoring the
tear trough

The tear trough is the groove that runs from the inner corner of the eye down towards the cheek. When volume is lost from this area — through ageing, weight loss, or genetics — the hollow casts a shadow that creates the appearance of dark circles, tiredness, and premature ageing, regardless of how well-rested you actually are.

Tear trough augmentation uses a very small amount of specifically formulated dermal filler to restore volume within this groove, softening the transition between the lower eyelid and the cheek. What causes

Genetics & Hereditary Anatomy

Some patients have a pronounced tear trough from a young age due to their bone structure and fat distribution. The groove is simply deeper or more defined in their anatomy — unrelated to ageing, and present regardless of sleep quality or lifestyle.

Age-Related Volume Loss

As the mid-face fat pads diminish and descend with age, the transition between the lower eyelid and cheek becomes increasingly hollow. The junction that was once smooth and well-supported becomes a visible groove casting a persistent shadow.

Weight Loss

Significant weight loss — including rapid loss from GLP-1 medications such as Ozempic or Wegovy — can deplete facial fat, making the tear trough hollow more pronounced even in younger patients who previously had little concern in this area.

Skin Thinning

The skin beneath the eye is the thinnest on the face. As it thins further with age and UV exposure, underlying structures including blood vessels become more visible — contributing to the darkened appearance alongside the physical hollow.

Full Facial Assessment

Dr Sabrina performs a comprehensive facial assessment before any treatment. This includes evaluating whether the dark circles are hollow-driven or pigment-driven, assessing mid-face volume, skin laxity in the lower eyelid, and confirming suitability for treatment.

Cannula Technique

Dr Sabrina uses a blunt-tipped cannula rather than a sharp needle. The blunt tip cannot pierce blood vessels — significantly reducing bruising and eliminating the risk of intravascular injection in the delicate periorbital zone. This is the gold-standard technique for tear trough treatment.

Precise Placement

The filler is carefully placed within the tear trough and massaged to achieve an even, natural-looking result. The procedure takes approximately 5 minutes per eye. Most patients continue with their normal day immediately after treatment.

A note on mid-face augmentation

Some patients will benefit from volume restoration in the mid-face cheek area alongside tear trough treatment. Tear trough filler is not suitable for patients with significant skin laxity or excess skin in the lower eyelid, those whose dark circles are primarily pigment-driven rather than hollow-driven, patients who are pregnant or breastfeeding, or anyone with active infection or inflammation at the treatment site.
